Drop a straight line somewhere near the middle of the heart shadow. Draw a line, as in "a," from the right heart border to that line. Draw another line from the left heart border furthest from the line in the middle of the heart shadow as in "b." Add the two lengths together. The cardiothoracic ratio is the ratio of the sum of lines a and b divided by the largest transverse diameter of the chest (not fully shown here).
