Each of the participating churches will host weekly a series of Family Care Management Support Group Sessions on topics related to health and legal issues for elderly and terminal ill family members. The Family Support Groups will be formed to assist family members that are responsible for the care of the older or terminal ill family member to deal effectively will health and legal issues. Topics to be discussed will be Living Wills, Power of Attorney and estate planning. At the end of the series of support group sessions, family care managers will have or have information about power of attorney, including a living will, an estate plan, and advanced directive for end-of-life determination for themselves and family members that they manage care for
WHERE Weekly Family Support Groups will be formed in four targeted Black Churches in the Greater 3rd Ward area.
WHEN Each participating church will determine dates and times for Family Support Groups
WHY - Many family care managers do not have legal documents that give them directives to the wishes and level of care their older family members desire for end of life determination. In an effort to reduce the stress level that family care managers experience, the series of family support groups will provide information that will assist them in accessing health and legal services for family members that they provide guidance for. We believe that these informative sessions will reduce the stress level of family care managers.
