    1. Enhancing Case Management through CQI Education for Adult Day Care services professional staff and HMO staff CAP
      1. One pilot center will track care outcomes and compare with standards and finally, communicate results
      2. EPOM will examine extent to which the Adult Day Care Centers employ CQI to improve programs

    1. Enhancing the Knowledge and Skills of Paraprofessional Service Providers Working in Elder Care CAP
      1. Assessment of needs, measurement of present knowledge & attitudes, measurement of clients needs and on-site training
      2. Content: psychosocial assessment, dementia, depression, Rx side effects, advanced directives
      3. EPOMS will measure knowledge and skills
    Texas Southern University, Houston – Maxine Hammonds-Smith
    1. Family Management System Project CAP
      1. Equip family care givers with case management skills to complement paraprofessionals
      2. Partnership with churches families and home health agency staff
